We will provide a professional photographer to take pictures of children in their costumes at affordable rates. Would you be interested in individual pictures of your child taken the day of the dress rehearsal? Please circle one. Yes or No
Recital Fee: There is an $19.00 recital fee this year per child. Families with more than one child in the show pay $19.00 for one child and $10.00 for each additional child. This is to pay for the planning, theatre space, decorations, props, and other expenses running the show. This fee does not cover admission into the recital.
Costume Fee: There is a $38.00 costume fee this year per child. If an individual child is in more than one dance, there is a $22.00 fee for each additional class. If your child's costume size is a XLC, SA, or any size larger, there is an additional $5.00 fee per costume. All costume fees cover costumes and shipping and will include tights for girls unless the costume has pants. Shoes will not be provided.
All fees and this form are due on January 7, 2012!
If they are turned in after this date, late fees and additional shipping fees may apply and we cannot guarantee the costume companies will be able to ship a costume in time for the show.

Registration forms can not be sent online. You can print the registration form and send or fax it to our office or use the one provided in the recital packet. If you are signing up for evening classes and do not have access to a printer, you can see your child's instructor or contact our office and we can send you one.
